Why does the first beat sound different?

The downbeat of each bar is accented with a higher-pitched, louder click so you can feel where each bar starts based on the time signature you choose.

Does the metronome need an internet connection or send audio anywhere?

No. The clicks are generated locally with the browser's Web Audio API. No audio files are downloaded and nothing is ever uploaded.
